A 2021 systematic review by Defined Exposure Assessment Working Group (published in Environment International) catalogued over 80 chemical compounds identified in secondhand vape aerosol.

Eighty. Not five. Not “trace amounts of flavouring.” Eighty distinct chemicals released into the air every time someone takes a hit.

The “water vapour” myth persists because the visible cloud dissipates quickly. Cigarette smoke lingers, yellows walls, sticks to clothing. Vape aerosol seems to vanish. But disappearing from sight and disappearing from the air are not the same thing.

Ultrafine particles — those smaller than 100 nanometres — can remain suspended in indoor air for minutes to hours after vaping. What’s Actually in Secondhand Vape Aerosol?

Nicotine. Yes, nicotine is present in exhaled vape aerosol. A 2019 study by Ballbè et al. published in Environmental Research found measurable nicotine on indoor surfaces and in airborne samples in homes where vaping occurred. The concentrations were lower than in smoking households — roughly 60–90% lower — but they were not zero. If you vape in a room with a child, that child is being passively exposed to nicotine.

Heavy metals. The heating coils in vape devices shed metallic particles. Goniewicz et al. (2018, Environmental Health Perspectives) detected lead, chromium, nickel, and manganese in aerosol samples. These metals don’t just affect the vaper — they enter the shared air.

Flavouring chemicals. Diacetyl and acetyl propionyl — linked to bronchiolitis obliterans (“popcorn lung”) in occupational settings — have been found in certain vape aerosols. Whether secondhand exposure reaches harmful concentrations is still being studied, but the presence of these chemicals in shared air is established.

What Does This Mean for Children?

This is where the research gets personal.

Children breathe faster than adults. Their lungs are still developing. Their bodies are smaller, meaning the same airborne concentration results in a higher dose per kilogram of body weight. And they have no say in whether the adults around them vape indoors.

A 2022 cross-sectional study by Braak et al. in Pediatrics found that children living in homes where adults vaped had measurable levels of cotinine (a nicotine metabolite) in their urine. These weren’t children who were vaping themselves. They were being exposed passively — through shared air.

The levels were lower than those found in children from smoking households. But they were significantly higher than in children from smoke-free, vape-free homes. Vape aerosol has fewer toxic compounds, lower concentrations of most measured substances, and no combustion byproducts. By most metrics, it is less harmful as a secondhand exposure.

But “less harmful than cigarette smoke” is an extraordinarily low bar. Asbestos is less harmful than a landmine. The relevant question isn’t whether secondhand vape is better than secondhand smoke. It’s whether it’s safe. And the honest answer is: we don’t know yet.
